app: add Issue Creation field

Often people are confused where to report a bug they experienced with their package. This clarifies the location of packaging issues and hopefully points people in the right direction.

A alternative approuch could be to give a link to the open issues of the package (simple search of origin), this doesn't need people to register a account, could avoid duplicate entries and helps people faster with their issue.
