setup-disk: NO_DISK_UUID for fstab

It can be helpful when creating an overlay to use /dev/... device names in /etc/fstab so the same overlay can be used across multiple devices. This MR allows an optional NO_DISK_UUID environment variable to override the default behaviour of using UUIDs.
